Machine Readable Passport

  • Fill out the application form
  • Copy of I-20
  • Copy of current passport
  • Passport size photo 1 copy attached to the application form. The size is 55x45mm. Resize and get the correct fit from www.epassportphoto.com. It can be printed in CVS or elsewhere.
  • Birth certificate copy (OR national certificate copy)
  • USD 110 money order or cashier Cheque.
  • Copy of the GMU/School ID card (Though not written in the spec. as requirement but the person there was trying to annoy me with this extra requirement)
